Design a Pamphlet and Membership ID for RoadStar

Contest Brief

We are looking for a freelancer to design the cover of a pamphlet for our customer contracts AND a membership ID. Our company is called RoadStar Coverage and we provide exterior coverage to customers vehicles. Requirements:

-Creative in Design but also professional (we are sending these out)
-8.5" x 5.5" in dimension (after it is folded in half).
-Must use gold and black in design (our company colors, but other colors can be used)

Attached is our logo and an example of another pamphlet we have for another company of ours. PLEASE DO NOT recreate the pamphlet for MVA, we need submissions for RoadStar that are different than the one we have now.

For the Membership ID, the front of the card has to relate to the pamphlet cover.
Requirements:
-Must be black and gold
-Must have section for this information: Name, Contract ID, Coverage, Customer Service, Claims, and Roadside.
-Must be double sided (the back can have all the information mentioned above)
-Front of the card MUST relate to the pamphlet.
-Membership card is the same size of an average ID or credit card.
